You have the right to choose United HomeCare for your Medicare home health care coverage after being discharged from the hospital in Miami-Dade County. Ask your doctor to write United HomeCare on your home health services referral form. It’s that easy!
Physicians with patients enrolled in Medicare or Medicare Advantage may prescribe the full range of services available. Services range from geriatric case management, skilled nursing care, physical or occupational therapy, home health aides, and medical social work services all delivered under the supervision of a Registered Nurse.
In addition, United HomeCare® participates in a number of programs to help caregivers of elderly clients. These include:
